POSITION SUMMARY

A Home Health Agency physical therapist is a professional practitioner who assumes responsibility for planning and providing treatment of physical and psychosocial dysfunctions in accordance with the physician’s plan of care; involves such activities such as, practice in vocational skills and activities of daily living. Directly supervises and evaluates Physical Therapist Assistant assigned. He/She can demonstrate his/her ability to make clinical judgments in an efficient and effective manner, without supervision. He/she can demonstrate critical thinking and performance ability in coordination of individual patient care needs. Consistently performs according to the standard of the profession.

CLINICAL RESPONSIBILITIES

Provides services and education/training to clients in their homes following the established Plan of Care under the direction of the client’s physician as evidence by:

• Compliance with OASIS protocol per Medicare and State regulations
• Performs Physical Therapy evaluation, makes recommendations regarding the patient’s level of functioning.
• Contributes to the development of the plan of care/treatment with both medical and nursing staff.
• Evaluates the effectiveness of current plan and client progress, recommends changes in treatment programs when needed and implement physician ordered changes.
• Physical therapist shall provide supervision and direction to the physical therapy assistant in the treatment of patients to whom the physical therapist is providing care. The physical therapy assistant shall be able to identify, and communicate with the physical therapist at all times during the course of treatment of a patient.
• Documents all services provided; observations, client progress and recommendations according to established plan of care in the client’s record and submits to Options for Rehab within 72 hours.
• Physical therapist shall sign each treatment provided by the physical therapist.
• Utilizes all equipment according to established procedures and reports defective equipment and safety hazards.
• Instructs patients, caregiver’s and appropriate home health care agency personnel regarding specific exercises for the individual patients, as appropriate
• Participates in patient/family education and regarding home health care and trains patient in accordance with home health agency policies and procedures. Participates and documents client case conference as requested to provide information to other members of the team. Physical therapist will participate in the discharge planning process in coordination with the physical therapist assistant.
• Serves as a member of the home care team

QUALIFICATIONS

• Successful completions of physical therapy program at a school of physical therapy approved by the American physical Therapy Association of council on Medical Education and Hospital of American Medical Association or the committee on Allied health Education and Accreditation of the American Medical Association.
• Current and unrestricted license to practice physical therapy in the State of CA.
• One year (1) of physical therapy experience preferred.
• Current CPR certification
• Must be familiar with general use and functions of the computer such as, user names and password concepts; internet; e-mail; navigation of computer desktop.

OTHER

• Licensed driver with automobile insured in accordance with state requirements.

• Participates and documents client case conference as requested to provide information to other members of the team.
• Submits documentation to agency as per guidelines.
• Physical therapy assistant utilizes physical means such as exercise, massage, heat, water, light and electricity under the supervision of the physical therapist as per physician’s orders to relieve pain, develop or restore function and maintain optimal performance.
• Physical therapy assistant will participate in the discharge planning process in coordination with the supervising physical therapist.
• Serves as a member of the home care team.

  • Perform measurement data collection or care prior to the evaluation of the patient by the Physical Therapist.
  • Document patient evaluation and re-evaluation.
  • Write a discharge summary
  • Establish or change a plan of care
  • Be the sole physical therapy representative in any meeting with other health care professionals where the patient’s plan of care is assessed or may be modified.

Occupational Therapist performs functions that require substantial specialized knowledge and skills. He/She can demonstrate her/his ability to make clinical judgments in an efficient and effective manner without supervision. He/She can demonstrate critical thinking and performance ability in coordination of individual client care needs. Consistently performs according to the standard of the profession.

POSITION SUMMARY

A Home Health Agency Speech Therapist is a professional practitioner that plans and provides Speech- Language therapy services to clients in their homes in accordance with the Plan of Care and under the direction of the client’s physician. The Speech-Language Therapist performs functions that require substantial specialized knowledge, judgment, and skill, must be able to make judgments accordingly.

CLINICAL RESPONSIBILITIES

Provides services and education/training to clients in their homes following the established Plan of Care and the specific functions identified by the Speech-Language Pathologist.

• Compliance with OASIS protocol per Medicare and State regulations
• Conducts appropriate evaluations, assesses home safety environment and identifies equipment needs relative to speech/language function.
• Identifies functional speech defects and establishes a plan of care to improve patient’s function.
• Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.
• Recommends communication devices/aides as indicated.
• Confers with referring physician and other organization personnel to ensure coordinated and comprehensive care and contributes to the development of the plan of care.
• Participates in patient/family education regarding home health care.
• Trains patient in accordance with home health agency policies and procedures.
• Accurately completes all necessary documentation on appropriate home care record/forms and submits to Options for Rehab within 72 hours.
• As appropriate participates in case conferences, inservices and other meetings required ensuring coordinated and comprehensive care.

QUALIFICATIONS

• Master degree in the field of Speech Pathology approved by the American Speech-Language-Hearing Association (ASHA).
• Master’s level professionals with a strong knowledge base in speech-language/literacy development and speech-language/literacy disorder, as well as a strong skill set in diagnosis, intervention, and workload management
• Current and unrestricted license to practice speech-language pathology in the State of CA.
• One (1) year experience of speech-language pathology experience preferred.
• Current CPR certification.
• Must be familiar with general use and functions of the computer such as user names and passwords concepts; internet; e-mail; navigation of computer desktop.
